School-of-Choice Forms

IDT/PUSD IntraDistrict Transfer Form: Completed forms can be turned in to the District Office, or for your convenience, MCHS' main office, beginning Feb. 1st. The earlier the better!  You will not be able to enter courses online until the District has approved your IDT. Please turn in your Course Request Worksheet to your student's Basic Ed teacher or directly to MCHS.  Indicate at the top of the form that you have submitted an IDT or write CHOICE.  We will enter your course selections for you.
